NEET exam is a common entrance exam for all Medical/Dental Colleges in India. The pattern of the exam includes questions from Physics, Chemistry and Biology, which makes these three subjects mandatory in order to appear for the exam.
Apart from PCB (Physics, Chemistry, Biology), you can opt for any other optional course of your choice, and NTA doesnot restrict you in doing so.
As long as you have an aggregate score of 65% in PCB subjects, you neednot worry about anything else(like your optional course).
