Yes , you will be eligible and can fill need mds application form if you lack a permanent registration number but have provisional registration number of DCI as to be eligible for neet mds candidates must hold BDS degree from a recognised institute to fulfil NEET MDS eligibility criteria 2020 and Provisional/permanent registration with State Dental Council or DCI by March 31, 2020. So , even if you have provisional registration number, you will be eligible.
