Hello,
you can get the application form from official telangana government web portal Meeseva. Here is the link :- https://www.telangana.gov.in/services/meeseva
you have to fill and submit it in the Meeseva kendra with required documents.
You can also do it from your local MeeSeva center. You have to ask for the application form and fill it up and provide the required documents. It will be done with in 1-2 weeks.
Basically, it is not fully online. You have to visit office to submit r collect documents.
