Assuming that you are taking admission through NEET Counselling, then the documents required are as follows:

• NEET UG 2021 Admit Card
• NEET  UG Result /Rank Card 2021
• Date of Birth Certificate
• Class 10th Certificate
• 12th Class Certificate
• Class 12th Marksheet
• Provisional Allotment letter
• Identity Proof (Aadhar Card/Voter Id,Ration Card,Passport, Driving Licence)
• Caste /PwD Certificate if applicable
• Eight(08)passport size photograph same as affixed on the application form
• Medical Certificate
• Character certificate

See first of all you require following documents for counselling process through which you will be allotted seats for the required course :-

1. NEET Admit Card
2. NEET Scorecard or Rank Letter
3. Class 10 certificate and mark sheet (for date of birth)
4. Class 12 certificate and mark sheet
5. ID proof (Aadhar/PAN Card/Driving License/Passport)
6. Eight passport size photographs
7. Provisional Allotment Letter
8. Caste Certificate (if applicable)
9. PwD Certificate (if applicable)

Now when you will be allotted a seat then also to the reporting center you have to carry the above documents along with the seat allotment letter which you will get after the seat gets allotted to you and all the certificates like Migration, passing, character and some other certificates that you got from the school where you had passed class 12th.
