DOCUMENTS REQUIRED FOR IIT JAM APPLICATION FORM :

• You should fill the your personal information, address for Communication, eligibility degree details, college name and address, JAM paper code, choice of examination cities
• Passport size photograph
• Image of your signature
• Scanned copy of category (SC/ST) Certificate ( if applicable) in PDF format
• Scanned copy of Pwd Certificate (if applicable) in PDF format
• Details of valid ID- You should carry original ID proof to the centre
• Net banking/credit card/debit card details for payment

Documents required to upload are photograph and signature
