Radhika, at this point in time, there is no official information regarding the dates for ESIC IP ward certificate. Please watch out for official information at esic.nic.in.Once the application forms are out, you need to download and fill it up and submit it at your nearest ESIC branch along with:
- 2 passport size photos of the Insured person
- ESI Card
- Copy of Proof of Dependant's benefit
- IP in receipt of 40% PDB or more
- For female Candidates, Copy of Affidavits
- 2 passport size photos of the Ward
- Copy of NEET Admit Card containing NEET Roll number
- Copy of Proof of Residence
- Copy of School Certificate
- Copy of SSC Certificate
- Caste Certificate if applicable

Ma'am there's a circular that has come today on 28.9.20 that says esic IP quota seats have been removed this year. Is this true??

You have to oppose this because they don't have the right to scrap the quota in last moment. You can use media or by filed a case against ESIC to restore the quota this year.
