how can I get declaration lieu of obc- ncl format for jee adv 2021

as for obc ncl certificate if your certificate is made after 1april 2021 then there is no any need of declaration form
but if your obc ncl certificate made before that then in that case you need to submit a declaration form which you download from jee advance website and fill it and then you need to upload it .
