I should tell you that a score of 540 can surely fetch you a government seat. Since you belong to the state of Andhra Pradesh you can get admission in Telengana only through state quota. Although the cutoff varies slightly every year let me give you the cutoff for some of the Colleges in Telengana:
ESIC Medical College Hyderabad - 445
Gandhi Medical College Secunderabad - 469
GMC Nizamabad - 417
