i changed my name in all id proofs but not changed in acedemics like Lata in acedemics and Lakshika in id's which nm i should use in gv form

As your query is your have changed your name Lakshika in all of your ID proofs whereas in your academic your name is still Lata .

I would suggest you, that it would be better for you to change your name from Lata to Lakshika in your academic even. So that you won't have to face any issue regarding the name change. Otherwise that may be a hurdle for you in your career .

Further, as if you want to go for Government form then it would be better to go with the name Lakshika , that is submitted in all of your ID proofs and that will be more way better option for you .

But if they request you to change your name in your academic then you should do it as soon as possible.
