For percentage of marks in 12th board what should I provide, the total percentage (in all subjects i.e. E,P,C,B,PhE) or only in E,P,C,B

The percentage will be as per your board calculation, whether it is best of 4 or best of 5, the way your percentage is calculated, for example in ISC XII it is best of 4.
