i forgot to apply for ward of ip certificate . but my father is insured person. is there any chance of getting the certificate now?
Hello! The Certificate will be issued by the regional/ Sub regional office under whose Jurisdiction the Insured person is currently working.The procedure for Issue of Ward Certificate-
The candidates need to approach the requisite RO/SRO after the issue of admission Notice in Pubic domain by ESIC, HQ office with an application form and self attested copies of the documents to be varified from the Originals given below.
ESIC Pehchan Card
NEET Admit Card containing Roll No.
Proof of Residence
School Certificate for proof of Date Birth/ Birth Certificate
Senior Secondry School certificate
