I have uploaded my photo whitout name and date of photo in neet application form. Is there any problem?

Yeah.. you may face lot of problem. And in extreme case your candidature might get rejected.
According to neet ug official notification , the pic must be in white background with the name of candidate and date of taking pic , which your pic doesn't follow the same. So i guess your application form might get rejected.
However you can talk to neet ug officials once, may be they could help in this regard.
