Documents Required

The following documents must be submitted by the candidates while filling up the application form:-

Aadhaar Card of Candidate
Annual Income Certificate
Domicile of Madhya Pradesh
Class 12th Marksheet
Proof of Admission
10th Scorecard and Certificates
Photo of Applicant
Complete Bank Details like Name of Bank, IFSC or MICR Code, Branch Name, etc
Contact Number

I have one doute that at time of admission SSSMID NO of all family is also required but my mother name in समग्र आईडी (SSMID)and in my Mark sheet is completely different
so in this case what I want to do
please help me
