If people who got qualifying marks don't make it in medical exam, will they approach who have got below cutoff and are medically fit?

No, there will be no such situation. It is mandatory to get atleast qualifying marks in NEET so as to be eligible for the admission process. Colleges will consider only those candidates who will get qualifying marks. And these candidates will be scanned for medical tests , if they fail , there are numerous candidates waiting in subsequent merit lists to pass medical test and get admission. But , it is necessary to qualify NEET.
