Yes, name and date on photograph is must. NTA did mention that in its information bulletin. But even if you did not mention your name and date on the photograph, but it follows the other rules like face is 80% visible. photo is clear and ears are seen, then your photo is fine only. You need not change the photograph and your application will not be rejected for this reason.
