The cutoff is subjected to variations every year depending on the number of candidates appearing for the examination,the difficulty level of the question paper and the reservation category of the candidates.
If we consider the previous years cutoff you seem to have no chance in government Medical college Of you belong to General category. But you can apply in private colleges.

First of all, you need to know that NEET is for a lot of courses like BDS, BAMS, BHMS etc, and not just for MBBS. If you are looking forward to an answer for MBBS, I must tell you that unfortunately I don't think that with a rank of over 20,000 you'll be able to get a good government MBBS college. But still you must look at your state rank which decides 85 percent of the seats quota. Also, there are other options like private colleges, management quota setas in deemed university etc which you can consider.
If you still wish to take admission in MBBS from a government college, you must take a drop and try to get a better rank, to be on the safer side, you must get 500-700 state rank.
