I have my obc ncl certificate with the same format given in the information brochure of jee advanced. But this is a state (west bengal) certificate. Do i need to make new certificate with the exact form given in the information brochure?

Hii tejvir,
Format of your certificate may differ from the prescribed format. That is not a problem at all.
But take care that it is a certificate provided and validated by central government and it should not be older than the date mentioned.
