My score is 392 in neet 2019 air is 136753 and category rank is 8500 can i get gov.clg.?

You have neither mentioned which category you belong to nor the state. With a score of 392, if your category is general or obc, it becomes difficult to secure a seat in the government medical colleges when comparing the results of previous years. Even if it is SC or St, you have chances only in a few states.
