My son has passed from delhi'if he wishes to go for counselling in j&k for medical seat with neet score do we have to fill seperate form

As your son has passed from Delhi, he is eligible for 85% quota of Delhi only. But if he has a domicile of J&K, he is also eligible for 85% state for J&K too. You have to fill the form for both the places separately.

In case of Delhi, he is only required to submit the class 12 marksheet. That would be enough to make him eligible for 85% quota of Delhi. But in case of J&K, he will need domicile certificate along with all supporting documents like residence proof , identity proof , etc.

Make sure to carry all the necessary documents with you at time of counselling
