Let's talk about government college admission through All India Quota first. Given that few colleges in AIQ have OBC category seats reserved for admissions, therefore despite having a fine score you may lurk on the verge of may or may not point when it comes to securing admission on government MBBS seat through AIQ. Now, coming to government college admission through state quota route. The chances are very high that with this score you get a government MBBS seat under OBC category in your state. However, it also depends upon the number of seats available in your state, aspirants competing with you and  much more.

In nutshell, I would say, there are chances for you to secure a government MBBS seat under OBC category in NEET 2018 with a score of 500 or so.
