Yes, You need to register seperately if you are applying for KCET for Engineering. The Medical admission will be through state counselling authority of Karnataka on the basis of NEET score, You need to apply when Karnataka MBBS/ BDS application form come out for medical programme & for engineering you need to do the registration separately for Karnataka.
