sir mymark is 500 and rank is 49000 in neetug 2019 [gen category]and also EWSis applicable for me.state UP. can i get any gov college

Yes , you will definitely get a MBBS seat under EWS reservation in any state and in a govt. College as the cutoff for EWS category is less in comparison to the Gen. Category.

As you mentioned with 49000 AIR, your chances of getting a decent government college in your state through state quota(85%) are negligible as previous year last student who got admission in government college had 24000 rank.
However you might get some good private medical college in your state through state quota.
As the admission to medical colleges based on NEET is given under All India Quota(15%) and state quota(85%).
