1. NEET admit card, Rank letter or AIQ Letter
2. Class 10 and 12 certificate and Mark sheet (for date of birth proof)
3. ID proof
4. 8 passport size photograph
5. Provisional allotment letter generated online after allotment
6. Community certificate if applicable
