Who is responsible for a company's Talent Management?

Everybody is. Although HR functions are responsible for developing career pathways and coordinating training programmes, company-wide engagement is required by a Talent Management strategy. Team leaders should recognise high-potential workers and discuss their needs for preparation. Then it's the responsibility of the Talent Management team (consisting of HR Generalists and Training Coordinators) to build personnel.
