Honestly i believe this is a very hypothetical question.
I see that you e clearly confused with what to take in your future.
You can't choose between an engineer and a doctor.
A doctor saves people and an engineer makes people.
It is completely on you whether you wanna take btech or mbbs.
Plus these two are not the only options to choose from. There are so many courses that you can do.
Please give it a decent thought.

Your future is what you make.
You will be able to make your or let us say life better when you choose what you are interested in.
However, there us no guarantee that choosing that particular stream will guarantee you a good life.
Your future is not solely dependant on what you choose now.
