As you know admission to government medical colleges is based on NEET marks is done under two categories-All India quota(15%) and state quota(85%).
With UR category and 454 marks, you might not get any government college through All India quota. But, you have high chances of getting decent government

The cut off marks for OBC category under All India Quota in Karnataka was 450 and above in 2018. The cut off marks change every year and therefore might increase or decrease. You can apply for the AIQ counselling but the chances of admission might be difficult.

According to 2018 statistics,  you might get a seat in any NIT or GFTI. And the local government colleges will not consider JEE Mains as the admission criteria. But there will be many private colleges that offer a admission and fee waiver based on the JEE Mains

You have got low marks and the cutoff has increased this year so you have very less chances of getting a government college. Though you can try management quota seat in self funded college you have chances of getting medical seat.
