Though you are doing BSc but you don't have physics in your bsc, so your highest qualification with all the three subjects i. e physics chemistry and Biology/biotechnology remains your 10+2
While, those who have passed 1st year of 3 year graduation with all the three subject i. e physics chemistry and Biology/Biotechnology needs to select code 5 and those who have completed BSc with at least any two of physics chemistry and Biology in their bsc need to select code 6.

--------) Code 1 :- code 1 is for class 12th board 2021 examination appearing students with physics, chemistry, biology and English or have have already appeared but results are awaited i. e have not been declared.
--------) Code 2 :- code 2 is for all those who have passed their 10+2/higher /senior secondary/ Indian School Certificate Examination with physics, chemistry, biology/biotechnology and English
--------) Code 3 :- code 3 says it is for those passed their intermediate / pre- degree examination with physics, chemistry, biology/biotechnology and English ,your physics, chemistry and biology/biotechnology should also contain practical exam.
--------) Code 4 :- pre - medical degree with Physics, chemistry and biology/biotechnology after passing hsc i. e 10+2 or puc or equivalent.
--------) Code 5 :- code 5 is for all those who have passed 1 st year 3 year graduation with all the three of Physics, chemistry and biology/biotechnology
--------) Code 6 :- code 6 is for all those students who have passed BSc with at least any two of following subjects :-

Your neet 2021 application/registration is divided in 2 parts:-

Government or private BAMS college is only allotted through NEET exam in Maharashtra.
Mhcet biology is not applicable for BAMS college in Maharashtra.
If you have appeared for neet exam this year then only you can think of getting admission in govt or private BAMS college.
There is no any other entrances exam valid in india for studying BAMS. Neet is only one way for doing that.
Govt BAMS college cut off is around 450+ marks in neet for Gen category candidate in Maharashtra state counselling.

Yes you can give neet exam next year too beacuse this year marks will not decide that you can sit next year or not and if you want to become doctor and you are determined so you should take drop and start preparing from today itself you should study your NCERT books and make notes which will help you while doing revision and you should solve neet sample papers and previous year papers too so you can get idea what type of questions are asked in exam and you would be aware of the level of exams

Eligibility criteria to apply for AFMC MBBS admission is that you must have passed 10+2 with Biology, Physics, Chemistry and English from a recognized board. You must have scored minimum 50% aggregate marks in English and 50% aggregate marks in Biology, Physics and Chemistry in each subject and 60% marks altogether in Biology, Physics and Chemistry. If the marks mentioned in each individual subject (i.e Biology, Physics and Chemistry) is out of 100 marks then you have scored less than 50% marks in Physics and Chemistry so you are not eligible to apply for AFMC.
You can use your original mark sheet for admissions. Improvement exam is for improving your previous scores if you couldn't improve it or scored less than the previous scores then your first and original marksheet can be used.

To be eligible for NDA examination, you need to pass 12th examination.
Coming to board% criteria/requirement,
There is no minimum percentage barrier in 12th to be eligible for NDA , all you need is to pass 12th.
Now,
You have biology as an additional / 6th subject in 12th , even if you have failed in it , you are still eligible for NDA, provided you should have overall your passed 12th with 5 subjects i. e your result status should be pass on your 12th marksheet/results .
