No. You cannot submit different board details from what you have submitted during the application form. It might create issues for you during counselling and document verification. Few authorities are ok with it but few authorities will be strict and you may have to face problems.

Get another application form and apply using it. There is no other way for it and if you lost the original copy then it won't be considered. If date is still valid and you can apply now also then apply it soon before the time lapse.
