There is no link between the CAT exam and the fee structure. The fee structure depends on the B School you choose and not the exam you give.

Yes , you will be eligibile . The CAT exam eligibility criteria says that you need to either be a graduate or in the last year of the graduation to be able to get into IIM. And most importantly , you need to have atleast 50% marks
Chances are less for admission to Government Medical Colleges under All India Quota and state quota. You may have chances in Private Colleges.

Ideally you should read two newspapers with one of them being a business daily. Hindustan Times is a good choice but it is highly recommended you read either The Hindu or The Indian Express. For business daily you can opt for either Financial Express or Business Standard or Economic Times.
There are lot of government colleges and each has different cut-offs. The cut-off for FMS would be 95 percentile for OBC, whereas it could drop to as low as 75 percentile for IIT Kanpur. So you understand the range of percentiles?
