• cancellationn request letter and application form for cancellation of allotted seat
• Payment receipt

The ews certificate is generally valid for a year, so if you issued your ews certificate on 23rd March, 2020 then it would just be valid for the year 2020-21. However I would recommend you to check the validity date of the certificate, it is usually written somewhere
