Well in that case you need a medical certificate from a government doctor about your disability and I think you should also discuss with the doctor or with the institute that whether this problem is considered as a disability or not.

No not all IIT won't accept your JEE mains score rather than this they will accept JEE Advance score. Mains score are only the cuttoff inorder to write the IIT-JEE Advance paper with that only you may be able to enter in these elite institutes.

Yes all vaccination have tobe taken which is mandatory as per mesical certificate norms by medical officers at IIT.

If students arenot vaccinated then they need to get done in first semester itself.

Yeah improvement exam will play a vital role in such cases because to take the elite institutes like IIT or NIT through IIT-JEE you need to score atleast 75% percentage in Class 12th. So go with that and yaah off course it will be beneficial for you.
