To be honest, with this score, it is difficult for a seat in MBBS in Government Medical Colleges. You might get a Deemed University at All India Level but in Jharkhand it is difficult. You will need 570 and more for a seat in MBBS even if you belong to

First of all with 75 percentile(considering your reservation) you may not be able to get a NIT but you can take a chance by waiting for the special round.However,that is uncertain. I will recommend you to try for SGFTIs(State Government Funded Technical Institute) by participating in counselling process

Despite having OBC reservation benefits, with 75 percentile in JEE Mains, you would not be able to get any NITs or IIITs for mechanical Engineering.
