As you have accepted and upgraded in round 3 COMEDK, your round 3 seat is safe. if you are not happy with the round 4 seat, you can just simply skip it as you will safely keep your round 3 seat . but if you accept the round 4 seat, you must join the college there's no option to cancel or get a refund and if you accept round 4 but don't report to the college, you'll lose your money and also have to pay penalty. so, unless and until you are fully sure about the higher fees, it will better to stay with your round 3 seat.

No, you cannot apply for Karnataka state quota. State quota seats are only for candidates who have domicile of that state or fulfil the state’s eligibility rules.
Since your domicile is Tamil Nadu, you are only eligible for Tamil Nadu state quota. For Karnataka, you can only apply through the All India Quota or management/NRI seats if available.

With 2,00,000 rank in NEET UG, even with a NCC C Certificate, it is very unlikely to get an MBBS seat in a Government Medical College (GMC) in India.
The National Cadet Corps (NCC) C Certificate does provide reservation and quota benefit in some states but the number of seats are extremely limited and usually gets filled under 70,000 rank.

If you complete your MBBS from another state through All India Quota, you are required to serve the bond in the same state where you did your MBBS and not your home state.
The bond is a state specific legal agreement between you and the state government where you are allotted your MBBS seat, so you will need to serve your bond in that state only.
