You can directly approach the college administration for management quota.
Every college has seperate limited seats reserved for management quota.

As you have scored 404 marks in Neet examination 2019 and you got 54000 rank in category 2A in Karnataka . According to you cutoff of government college is 380 marks of 2A category and you haven't mentioned the name of college. If it is 380 marks, then
As you have filled OBC-NCL category during filling of NEET application and during counselling you need to show that you belong to NCL. NCL means Non creamy layer , a person with income less than 6 lakhs comes under NCL category. You need to produce this during counselling.

According to your marks, chances would be tough o get admission in government medical colleges through All India Quota. You can try in private medical colleges and Deemed Universities.
