As per your DoB you will be of 19 years old by September 2022 and your DoB does not lie in the period of January 2, 2004 to January 1, 2007 so according to your age and DoB you will not be eligible for the NDA 2 2022 , so you cannot appear for NDA 2 2022.

>>> Indian Army - Should have passed 12th standard from valid State Education Board or a University.
>>> Indian Air Force and Naval - Should have passed 12th standard with Physics and Mathematics from valid State Education Board or a University.

As per your DoB you will be of 18 years old by January 2023 and also your DoB lies in the period of January 2, 2005 to January 1, 2008 so according to your age you will be eligible for the NDA 2 2023 , so you can appear for NDA 2 2023 .

No you cannot apply for NDA because as per the eligibility criteria for NDA, permitted limit for Myopia is -0.75 but you have -4 so you have exceeded the limit, therefore you will not be eligible neither for Indian Army nor for Indian Navy as well as Air force.

No there is no any reservation in NDA neither for wards of ex-servicemen nor for any other categories.
If you want to appear for NDA then you should satisfy the given eligibility conditions :-

Yes you would be eligible for entry into Indian Air force through NDA examination because you meet the required criteria. As per the rules a candidate needs to have Physics, Chemistry, and Mathematics as his/her subject in senior secondary education to be eligible for the air force and Navy wings of NDA while mathematics is required for entry into Indian military. But I would want to tell there would be no relaxations or anything such for you would have to clear the required cutoffs for the mathematics as well as general aptitude section along with the overall cut off.

To appear for NDA 1 2023 you must be born in between July 2, 2004 and July 1, 2007. To appear for NDA 2 2023 you must be born in between January 2, 2005 and January 1, 2008. Considering the date of birth criteria you are eligible to appear for NDA 1 2023 only provided you must have passed/appearing in 10+2 examination from a recognised board. You are also eligible to appear for NDA 2 2022 as you will pass class 12th examination in 2022.
For the SSB interview stage of the NDA selection process , you must carry the following documents:
SSB call letter
A valid photo ID
Original 10th pass certificate (To ascertain DOB)
Original 10+2 pass certificate or equivalent (To ascertain educational Qualification)
Caste certificate if applicable

For written you just have to rely on 11th and 12th cbse syllabus, ncert books solely and a bit of current affairs at that time . You should have the detailed syllabus for NDA exam, and you can match it with cbse 10+2 syllabus. For ssb you should contact any ssb preparation centre after clearing written exam.
