The dates for exercising web options for admission to UG MBBS/BDS courses will be notified separately on University website. See PwD reservation is the horizontal type reservation not vertical so it gets 5% advantage along with the other Unreserved/Reserved categories as per the Rights of Persons with Disability Act, 2016 but to be considered for PwD reservation you should have at least 40% of disability and as you have mentioned that you have only 18% of disability then you are not eligible for PwD reservation across any category.
Since you have not mentioned your home state so considering it as UP according to your profile and category as general.
So on such marks and category you have good chances of getting some Private medical colleges in UP under state quota according to previous year's cut-off. As we go through previous year's cut-off then it shows that you can get following colleges :-

NEET cut off is different for all colleges, I'm listing colleges with state quota cut off rank
Also as you haven't mentioned your category, I'm assuming it as general
1. American Institute of medical sciences: 191251 rank and 346 score
2. Ananta Institute of medical sciences: 263732 rank and 296 score
3. Dr SN medical college: 4225 rank and 619 score
4. Geetanjali medical college: 361511 rank and 244 score
Note: Cut off varies every year depending upon no of students appearing, no of seats available, last year's trends, difficulty level of exam and so on.

Every year the admission in government college differ according to the cutoff and the difficulty level of the exam and also on the total number of candidates appeared in the exam.
According to your score it is very difficult to predict either you will get chance for the admission in government mbbs college or not.
If you want mbbs then you can apply in private colleges from state quota or from management quota
you can also apply in Dental colleges for BDS
