NTA percentile system takes a lot into consideration than just total score. They have an order of preference while deciding the rank
1. Candidate scoring higher percentile in first subject of the question paper either physics or mathematics gets the highest percentile
2. Candidates who has attempted all three sections gets more percentile than candidate who leaves one section untouched or doesn't answer a lot from a particular section
3. The importance of subjects goes in a descending order i.e, first subject on the screen has highest priority
All of these and some other factors decide the rank that may be why your friend got a better rank even though he scored less. Hope this helps.

You do not have a high enough AIR rank, but with your category rank in NEET UG, you have a fair chance at getting into the MBBS and BDS Colleges in Tamil Nadu via the State Quota Seats (85%) but a low chance with the All India Quota Seats (15%).
Even though you may not get into the deemed government institutions, you can apply to the private colleges for MBBS and BDS programs and see how that works out for you. According to the category rank you have mentioned in the question you cannot get any of the NITs until and unless you deserves Home state quota as per previous year's cut-offs but I would suggest you to sit for some state counselling because in some of the state government colleges you can get admission on such ranks.
Note that the cut-offs are bound to change every year depending upon various factors.

Your JEE Mains score is valid only to get you admitted into a private college but for government engineering colleges your boards marks will be considered.So with a 68 NTA score you can apply to colleges like Charusat (Changa),SVIT Vasad, Parul University, Navrachana University,etc.will be your top 4 college choices.So best of luck.

Your score is your marks itself, on the basis of your score your rank is calculated, and if you belong to general or obc category then unfortunately you would not get into any nit, iiit other good college.
You may get some low ranked regional college bit that won't guide you towards a better future.
