You have not mentioned your state. Anysays, with 1.21 lakh rank, your chances of getting a decent government college in your state through state quota(85%) are negligible as cutoffs are usually much lower for general category.
However you might get some good private medical college in your state through state quota.
As the admission to medical colleges based on NEET is given under All India Quota(15%) and state quota(85%).
Having said that cutoffs change every year depending upon level of toughness of exam. So hope for the best and follow the counselling process.

It purely depends on your state rank, no. of seats in that state, your category.
No one can predict it without knowing all this please atleast specify your state.
Some states like-
- Karnataka
- Gujrat
- Maharastra
- Puducherry
- Tamil Nadu
- (May be Andra and Telangana too)
have ample no of seats and the cut off in these states are very low compared to other states.Inform if you get any.
Other states like-
- Delhi
- Bihar
- rajasthan
- Madhya Pradesh
Had high cutoffs last year so its difficult to get something there. (general category)
