Self attestation is basically your signature in blue ink on the xeroxed copy of the original document. anyways make sure you bring proper original and attested copies of the documents during the counselling or admission process. Documents submited online are often not scrutinized that rigorously so it should not be a problem at this stage

You don't need to panic about this situation. All you have to do is email their office providing your application details and request for editing fhe form
