As per the information you have shared, you will have to select Code 5 as your Qualifying Examination Code in the NEET application form.
According to the information given for Code 5, candidates should be in "the first year of the three year degree course of a recognised University with Physics, Chemistry and Biology/Bio-technology including practical tests in these subjects provided the examination is a University Examination and candidate has passed the earlier qualifying examination with Physics, Chemistry, Biology/Bio-technology with English at a level not less than a core course”.
What this simply means is that if your first year BSc includes Physics, Chemistry and Biology/Biotechnology with practicals, then the marks you secure in the first year final exam will be the qualifying marks. Equally importantly, you should also have previously cleared your Class XII with Physics, Chemistry, Biology and English as core subjects from a regular board.
So since you have not completed your BSc and are only in the first year, and assuming that you are studying each of the above mentioned subjects, Code 5 will be applicable.
