In the present case as yours, proforma b will be applicable. The meaning of different proformas are :
Proforma a1 : resident of state for 10 yrs continuously to be signed by Councillor/magistrate etc.
Proforma a2 : resident of state for 10 yrs continuously to be signed by Principal of school.
Proforma b : parents are a resident of the state for 10 yrs continuously to be signed by Councillor/ magistrate etc.
