As per Code 3 of the listed qualifying exam codes in the NEET application form, candidates could have cleared “The Intermediate/Pre-degree Examination in Science of an Indian University/Board or other recognised examining body with Physics, Chemistry, Biology /Bio-technology (which shall include practical test in these subjects) and also English as a compulsory subject.”

Code 3 is therefore applicable to PUC students who have already cleared their exams and have their Class XII marks available with them. Since you cleared your PUC in 2017 and are attempting NEET for the second time, Code 3 will be the qualification code applicable to you.
