The cutoff under All india Quota in Andhra Pradesh in 2018 was 539 marks(general category).
So it is evident that you donot have any chance of getting government medical college even if the cutoff decrease slightly. With a score of 447, getting a seat even in government dental college is next to impossible.

You have not mentioned whether you belong to general or any other category. If you belong to general catgeoy then forget of getting a seat in government college through All India Quota. You cannot get a seat for both BDS and MBBS in government college with this score. If you belong to SC/ST category then you can try. Last year through All India Quota many Colleges had cutoff less than your score. So you can try. But you can get MBBS in private colleges. So you can try for that also.
