I have different name in neet admit card and 12 marksheet (my actual name) which name should i enter in neet counseling application?

Brother then why should you enter different name in your NEET admit card.
Actually in neet application, the asked name is my name in aadhar card it is japesh selvaraj...but my name is japesh os which used for all certificates from birth certificates....
