if i have caste certificate of Uttar Pradesh but obc ncl with central list then i can put in aiims paramedical form or not..?

is central government sign necessary for obc ncl central form or it can be signed by sdm also as written in form. I have filled manually by hand( I have state obc caste certificate with me). is manual form acceptable in neet. pls tell full info. iam giving neet/aiims exam next month without coachin)
