The following documents are required at the time of document verification during NEET counselling in Uttar Pradesh:
NEET 2019 Admit Card
NEET 2019 Rank Card
10th Mark Sheet & Passing Certificate
12th Mark Sheet & Passing Certificate
Category Certificate
Domicile Certificate
Character Certificate
Migration Certificate
Passport Size Photographs
Printout of Registration Form
Allotment Letter
You also need to submit a bank draft of security fee in favour of Director General,Medical Education and Training,UP,payable only at Lucknow. Only CTS bank drafts are accepted.

Class XII Certificate
Class XII Marksheet
8 passport sized photographs (Same as the one affixed on the application form)
Provisional Allotment letter generated online
Proof of Identity
Category Certificate (if applicable)
