NEET -UG FORM Which will be my residence address ..if I was born in Lucknow ,studied from 1 to 2 in Patna and completed my 12th from Delhi

For NEET UG form, you can fill any address from these States as per your current address which should match with your address mentioned in Aadhar Card and you will be eligible for 15% All India Quota seats.

As you mentioned you lived in Bihar, Uttar Pradesh, and Delhi, the eligibility of all these states for 85% State Quota seats is different.

For Delhi, you need to complete 11th and 12th standard through regular classes from a recognized school within the National Capital Territory of Delhi.

For UP, you should have a domicile certificate, if you have not completed 11th and 12th from UP state.

For Bihar, if your parent/s are a domicile of Bihar state or they work for Bihar government then you are eligible for 85% Bihar State Quota seats.
