As you mentioned, you have completed Class 12 in the year 2018. In this case, you are required to submit Code 2 as the Qualifications and Qualifying Examination Codes. Since, you did not pursue BSc Physics further, you cannot fill Code 5. This code can only be filled if you have completed at least one year of the degree course by qualifying the University examination. Therefore, Code 2 will be applicable in your case. The last qualifying examination will be considered Class 12 with Physics, Chemistry, Biology/Biotechnology and English.
In addition, the minimum required marks in the science subjects (PCB) should be at least 50% if belonging to the unreserved category (40% for reserved category and 45% for PwD category). Scoring passing marks in the science subjects and English individually is also required.

The codes which are given by NTA are very difficult to understand.
Here I simplified the codes
Code 1 - 12th students giving their boards in 2019
Code 2 - 12th passed out students 2018 without gap in between 11th and 12th class
Code 3- 12th passed out students with gap in between 11 and 12th class
Code 4- dropper students ( more than 2 drops)
Code5- first year students of BSC with physics chemistry and biology/biotechnology
Code6- BSC passed out students
Code 7 - for NRI students
According to this simple version of the codes you can identify which code is for you

CODE 5:(The first year of the three years’ degree course of a recognised University with Physics, Chemistry and Biology/Bio-technology including practical tests in these subjects provided the examination is a University Examination and candidate has passed the earlier qualifying examination with Physics, Chemistry, Biology/ Biotechnology with English at a level not less than a core course.)
If you are 2018 passout you should give yourself chance to completely prepare for 1 year for neet.Let me tell you whatever you have read in school ,neet questions will not be same .So you should devote entirely for neet.Be focused ,Be motivated and be regular .At last go for the test series and analyse your mistakes and improve yourself.
