If you are talking about anti ragging afidavit that needs to be signed, you can download it from the State Counselling registration websites. There you will have a downloadble format of these proformas. You need to duly sign it and submit it while  reporting to college. If you cannot find one there, you can fill it in the college campus itself by asking them for a proforma.
