In some boards they use different way of writing the student's name, in some first name is written at first followed by the surname while in the other the surname might be written first followed by the name. Both the forms are correct and will not cause any problems to you in future. Just make sure that the spelling is correct in both aadhar card and 10th class marksheet otherwise that might cause a problem.

Candidate you cannot use the non -updated aardhar card for JEE because the website doesn't consider it during registration itself.But,it is actually not mandatory to apply with Aadhaar Card. You can also use your Passport no. , Bank Account ID or any other valid Government ID as a proof.

Yes of course you can use your ID card issued by your school because it is acceptable by NTA as you Identification type. For this you have to select the "School ID card" option in the drop down menu of Identification type in the application form of JEE-Main 2021 then you will have to enter your School ID card number in just next column.
Note that this application process will continues till January 16th 2021 and last date for payment of application fee is January 17th 2021 after which neither application nor application fee will be accepted and then after the correction window will be opened for the applicants from January 19th to January 21st 2021. Admit cards will be available for download from second week of February 2021 and the examination will be conducted from February 23rd to February 26th 2021 in two shifts per day, for February attempt.
A candidate will also have option to apply for one Session or more than one Session (February/ March /April /May 2021) together and pay exam fee accordingly. In other words, if a candidate wishes to apply only for one Session, he / she has to pay Examination Fee only for that Session during the current application period and will have opportunity to apply again for the March/ April / May Sessions.

Some insights of JEE-Main 2021 :-
> To qualify for JEE Mains, there is no minimum aggregate percentage criteria but yes if you want to take admission into NITs, IIITs or CFTIs then you should score a minimum aggregate of 75% in your class 12th if you belongs to General/Unreserved category and 65% if you belongs to any of the reserved category.
> Also from this year according to New Education Policy the JEE-Main examination will be conducted in English, Hindi and other regional languages such as Bengali, Gujarati, Assamese, Kannada, Marathi, Punjabi, Tamil, Telugu, Urdu, Odia and Malayalam.
> In JEE-Main 2021 there will be total 90 question out of which you have to attempt only 75 (25 from each section) and there will be two sections for each subject, Section-A will be of Multiple-Choice Questions (MCQs)(20 compulsory questions) and Section-B will contain Questions whose answers are to be filled in as a numerical value (10 numerical based questions out of which only 5 are compulsory to be attempted).
> In Section B, candidates have to attempt any five questions out of 10. There will be no negative marking for Section B.
> All question will be of 4 marks and in MCQs there will be negative marking of (-1) on incorrect attempt.

As long as your information is correct in the aadhar card then you will have no problem with your old aadhar card but if by update you mean updating information like address then i will suggest you to use some other government id with correct information on it for competitive exams. If by updating you me updating photo in aadhar or email id or phone number then you need not worry it won't cause any problem in the competitive exams.
See after passing class 10th, usually your class 10th marksheet is considered as your Birth certificate so wherever required you can submit your class 10th marksheet as your birth certificate but if anywhere your actual birth certificate will be demanded then you can use an affidavit with it regarding the correct name of your Father. As you have mentioned that for medical counselling you are concerned so during medical counselling your DOB certificate will be considered as your class 10th marksheet so you don't have to worry for it but if still you wants to resolve the issue of your Birth certificate then you can use a valid affidavit with it.
The scholarship scheme of Uttar Pradesh government is for each and every category and the application process for it was ended on December 15th 2020. Now the point which you have mentioned is not going to create you any problem because in the brochure released by UP Government for this scholarship scheme, it is clearly mentioned that the mobile number which you are giving should be linked to your bank account and your bank account should be linked with your AADHAR card so it does not means that your AADAHR card should be linked to the same number from which your bank account is linked.
In the case if you have submitted the number which is not linked with your bank account in which you want to receive the amount of scholarship then it might create a problem, so if it will create any problem then you will receive the mail from the competent authorities regarding the issue and then you can improve it but as of now the application window is closed and now you cannot edit your details.
